Agewell’s Thinking Differently About Dementia Project is pleased to be leading a pilot roll out of the My Future Care Handbook, which is a 60-page handbook, packed with the sort of information and guidance we all need to help us plan in later life.

It can be used for simply recording our everyday needs and wishes to also helping to plan longer term arrangements around care and finances for example, with tips and contact details for organisations and professionals.

Developed by Zoe Harris of Community Interest Company, My Care Matters, from her personal experience as a family carer, we have some limited copies available for FREE for people living with mild to moderate dementia who are Sandwell residents or are registered with a Sandwell GP.

We can support families through the use of the handbook and also offer advice and information on our other services.

Sandwell Community Dementia Service (SCDS) is supporting us on the pilot rollout and they take referrals from families in Sandwell who are living with dementia at every stage from awaiting diagnosis onwards.
