Our Partnerships

Agewell has a proven track record in delivering services which enable older people to live happier, healthier lives, retaining their independence for as long as possible.

These services are delivered by our staff and volunteers, and by working in partnership with other like-minded organisations, including those listed below.

Dementia Pathfinders CIC

Dementia Pathfinders is a social enterprise which delivers education and learning for people working in the dementia care field and provides therapeutic care and support for people with dementia and their families.

Since April 2017, Agewell and Dementia Pathfinders have been working together to deliver the Kingfisher Buddies service, which supports those living with dementia in Sandwell, and their family carers, through home visits and weekly groups. Dementia training is also provided.

West Bromwich African Caribbean Resource Centre (WBACRC)

WBACRC is an established community-based charity based in West Bromwich.

Their prime purpose is to improve the life chances, opportunities and experiences of African Caribbean communities and local people and they provide a range of health, social, educational and information services to all life stages (from birth to grave).  

They have been working in partnership with Agewell since April 2017, helping to deliver the Sapphire Service which provides hospital and community support for both the person in hospital and their family carers prior to discharge and once the person returns home, supporting the discharge process, and aiming to bridge the gap between health and community services and reduce readmissions.

Murray Hall Community Trust

Murray Hall Community Trust is an established charity based in Tipton, West Midlands.

They work with children, families, young people and adults to make a positive impact on their health and wellbeing, reduce isolation and loneliness, facilitate empowerment to ensure people get the right support and help individuals to maximize their potential.

Since September 2018, Murray Hall and Agewell have been working together to deliver the SPARCS service, which supports people over 65 years old in Sandwell, to maintain their health and wellbeing and to reduce social isolation and loneliness.

The aim is to help people to stay well and healthy in their own homes, to stay connected and to be able to access support when needed within their own community.

St Albans Community Association

St Albans Community Association is a Smethwick-based charity providing access to inclusive and supportive services to all ages.

Since April 2019, they have been working in partnership with Agewell to help deliver a Dementia Support programme in Primary Care Settings (GPs) for people recently diagnosed and their family carers. It also offers a home visiting service for up to 6 weeks.

Crossroads Care

Crossroads Care provides a wide range of regulated and none regulated community care services, to carers and the people for whom they care, throughout Birmingham, Sandwell, Walsall, Wolverhampton and surrounding areas.

They provide respite care or advice which enables families to carry on caring for their loved one within their own home.

Agewell, Dementia Pathfinders CIC and Crossroads Care are working in partnership to deliver a Dementia Navigators service across Sandwell for peope living with dementia and their families and have developed the Sandwell Dementia Roadmap, which is an informative online resource where you will find high quality information about living well with Dementia in Sandwell.   .

Communities In Sync (CIS)

Communties In Sync (CIS) is a Charitable Community Benefit Society based in the West Midlands whose primary objective is to improve the well-being of local people, especially the most vulnerable, through the provision of high quality, responsive services and initiatives.

CIS creates a single point of contracting with the responsibility for setting up and managing sub-contracts with individual consortium member organisations. The combined turnover of the member organisations is curently over £7 million with a frontline staff-force in excess of 350 employees.
